Patients

Wounds are ultimately affected by your overall health status. If you wish to optimize your wound's ability to heal, then it is best to control your current health conditons to give your body an optimal environment, and each wound needs a proper dressing. To better assist in this healing process, I encourage you to make an appointment for a face-to-face evaluation with Paradigm Wound Specialists.

Paradigm promotes patient autonomy. The patient has a right to see whichever provider they desire-- and sometimes that means collaborating with multiple providers. We are willing to work with each person and their current provider team to form a patient-centered treatment plan for their wound. We want to know your goals, so that we can better assist you in achieving them.

Due to the large radius of coverage within and outside of Tulsa, we are unable to see patients on a daily routine. However, our providers and staff will work with your current insurance to help you obtain in-home healthcare. Sometimes this means getting home health involved, which is often a covered resource under many insurance plans. We can help navigate this for patients after an initial evaluation..

If you have an open wound, we will manage any of your primary needs that affect the wound.

We can visit you on any day if your wound needs treatment. Ideally, we want to provide treatment on alternate days as home health. This will provide you with wound care throughout the entire week. We collaborate and communicate scheduling with your home health team.

We do accept patients without a PCP. Once we see the patient, we encourage them to establish with a PCP. We can cover their primary needs until they are able to establish with a PCP.

We currently cover 200+ mile radius outside of Tulsa and even have clients as far as Weatherford and Mcalester. If there is ever a question, please send us the clinicals and give us a call. We have taken care of patients outside of our coverage area many times before.

Yes we do. We have a flat rate for evaluation and incorporate distance traveled per the standard rate. All other procedures and advanced modalities are discussed on a case-by-case basis, but are generally priced at the rate of the CMS’ physician-fee schedule.

We are the only mobile wound clinic that staffs nurses for nurse visits. We assist patients with simple dressing changes if they are unable to change their own dressing. Our providers are all vetted heavily, with prior wound care experience before working. We also accept multiple insurances, whereas other clinics are looking for Medicare-only.

Yes. We manage, order, change, discontinue, and continue wound vacs.

We can treat anywhere that the patient resides. The only exception is Day Centers, hotel/motel(s), and unsafe living spaces. We are currently treating patients in homes, assisted living, independent assisted living, nursing homes, skilled nursing facilities, inpatient rehab, and LTAC settings.
Providers

We are able to visit on the same day but we make every effort to communicate with the home health involved so that the patient can receive maximum coverage throughout the week. We are a traveling brick-and-mortal clinic so we do not bill the same as home health. Our billing and visits do not interfere with home health.

Since we are a private clinic, we can see the patient as many days as medically-necessary. A MWF dressing does not intimidate us. If it is a wound vac, we will assess the wound to see if the vac change frequency can be decreased to 2x's per week instead of MWF.

Home Health

We can assist with primary care services, as long as the patient has an open wound, and we will assist with the 485 form. We will attempt to find the patient a communal PCP after the initial work-up.

We are able to provide a second assessment and treatment plan if the wound is not experiencing positive outcomes. We encourage patient autonomy and respect our communal colleagues. We will reach out to the current provider to discuss our treatment plan. Oftentimes, a collaborative approach is necessary.

We accept referrals via fax, phone, secure email, or via EPIC. We have a pdf referral form that is fillable. Our referral team is clinical, so please do not hesitate to call with any clinical concerns, questions, or to further discuss the discharge.
